Modern Pediatric Dentistry begins with an introduction to the field and then guides students through the recent advances and discusses different dental conditions found in children and the various methods of treatment.
One method of seeing how pediatrie dentistry is practiced is to take a "walk" through a modern pediatric dental office for a dental examination and for subsequent restorative care.
INITIAL DENTAL EXAMINATION
The most common ape for seeing the child for the first routine dental examination is 3 years. However, many pediatrie dentists advocate seeing the child for the first time at ages 6 to 12 months.1 At this appointment the parent can be given instructions on preventive care for the primary teeth which are starting to erupt. The child may be seen again at 2 years to reinforce preventive care to the parent and to introduce the child to the dental environment.2
When a 3-year-old patient and his or her parent enters the door of a pediatrie dental office, the environment should pur the child and parents at ease. It is similar to the modern pediatrie office with play areas and a "home-like" atmosphere (Figure IA & B).
The initial visit is a diagnostic appointment involving four steps: 1) obtaining a full written health history from the patient, 2) interviewing the parent and child to supplement the questionnaire, 3) performing an extra- and intraoral examination of the patient, and 4) taking radiographs, models and other tests. 3 The parent completes the medical/dental health questionnaire upon arrival at the office. The pediatrìe dentist supplements the questionnaire by interviewing the parent and patient to obtain additional information. The interview gives the pediatrie dentist the opportunity to establish a rapport with both the child and parent.
SEPARATION OF CHILD FROM PARENT
When the interview is completed, the child and pediatrie dentist leave the reception room or interviewing room and enter the dental operatory area. If the child is 3 years or older, the parent does not accompany the child to this area in most pediatrie dental offices.
In a survey of 120 members of the Association of Pedodontic Diplomates, only five respondents indicated they always allowed the parent to accompany the child into the operatory, 97 allowed the parent to accompany the child in selected cases, and 18 never allowed the parent into the operatory during treatment.4 Generally, those "selected" cases are children under 3 years of age or handicapped children.
Separation of the parent from the child is not a new technique. Belcher,5 in 1898, discussed the merit of excluding the parent from the operatory and how it aided in controlling the child's behavior. Starkey6 has suggested that the policy of requiring the parent to remain in the reception room can be justified for the following reasons:
1. The parent often repeats orders, creating an annoyance for both the dentist and child.
2. The parent often injects orders, becoming a barrier to the development of rapport between the dentist and the child,
3. The dentist is unable to use voice intonation in the presence of the parent because he/she may become offended.
4. The child divides attention between the parent and the dentist.
The needles used in pediatrie dentistry are disposable and should be no longer than 1 inch in length and are 25, 27, or 30 gauge in diameter. The anesthetic solution is injected slowly because a rapidly expelled solution causes discomfort.
The most common injections are regional infiltrations and nerve blocks. A recent innovation in pediatrie denral anesthesia has been the intraligamental injection. The needle is positioned into the periodontal ligament and the anesthetic solution is expelled under pressure into the tissues. This technique avoids unnecessary soft tissue anesthesia and possible subsequent self-inflicted postoperative trauma. By taking the time to adequately prepare the patient and by using a careful…
